Official Rules

NO PURCHASE NECESSARY TO ENTER OR WIN
The M&I Bank Scholar Award for Student Athletes runs from August 6^th – November 30^th 2007; The Award is sponsored by M&I Bank and KTVK their subsidiaries, parent companies, and affiliates (Sponsor). 

ELIGIBILITY:
Open to all legal residents of Arizona, 18 years of age or younger, must be a senior (12th grade) in an Arizona High School.

Criteria: Student must have a GPA of 3.25 on a 4.0 scale and must participate in at least one season within a particular sport. Void where prohibited by law. Employees of Sponsor, its parent corporation, affiliates, subsidiaries, advertising and promotion agencies, and their dependents and immediate families (spouse, parents, siblings, and children) are not eligible to enter or win.

HOW to ENTER and WIN:
To enter, go to any participating M&I Bank location to pick up your entry packet. Fill out your packet, submit required material and mail or drop it off to: M&I Bank Attn: AZ098-4027 PO BOX 56969 Phoenix, AZ 85079-9953. (As an alternate method please send a self addressed, envelope with postage to the above address to receive your packet)

The entry period begins August 6, 2007 and ends October 27, 2007. (All entries must be received by October 27, 2007.)

Watch Arizona Tonight on Fridays on KTVK from 10pm-11pm from September 14 – November 16 where a winner will be announced each Friday night. Each of the 10 winners will be highlighted on HSGameTime.com from September 14 – December 7, 2007.

The Grand Prize winner will be announced on Friday, December 1, 2006. Only one (1) entry per person or household during the Entry Period will be eligible for the Award, as determined by name, address and/or unique email address. All entries must be co-signed by your legal guardian.

ENTRIES:
All entries become the property of the Sponsors and will not be returned. The Sponsors are not responsible for lost, late, misdirected or delayed entries or entries not received for any other reason, or for errors in these Official Rules. Sponsors reserve the right to correct any and all printing, typographical, or mechanical errors. All winners must be qualified as meeting the eligibility requirements identified above at the time of redemption.

PRIZE(S):
Ten (10) winners will receive: A $500 check for the use of educational and related expenses from M&I Bank.
Total retail value: $500

One (1) Grand Prize Winner will win: A $5,000 check for the use of educational and related expenses from M&I Bank will be awarded on or around April 15, 2006. Grand Prize will be given to the institution the student will be attending on behalf of the winning student. Approx. retail value: $5,000.

Prizes must be used as specified by participating Contest Sponsors and are subject to all restrictions and conditions of the Contest Sponsors. All taxes are the sole responsibility of the winners. An IRS Form 1099 will be issued to winners of the Prizes. The winner must provide identifying information for proof of eligibility upon request. Each winner may be required to execute and return an Affidavit of Eligibility and a Liability/Publicity Release within fourteen (14) days following attempted notification of such winner. By participating in this promotion, entrants agree to be bound by the Official Rules and the decisions of the judges. Contest Sponsors are not responsible for any typographical or other error in the printing of the offer, administration of the sweepstakes or in the announcement of the prizes.

WINNER SELECTION:
The 10 winners will be chosen by a panel of judges from M&I Bank. Criteria: Student must have a GPA of 3.25 on a 4.0 scale and must participate in at least one season within a particular sport. Grand Prize Winner will be chosen by the M&I judges and two members of the community from among the 10 winners picked by M&I Bank judges during each Entry Period. Judging will be made by the Sponsors, whose decision is final with respect to all matters related to the Sweepstakes. Selection is subject to verification of eligibility and compliance with conditions.

NOTIFICATION OF WINNERS:
The winner will be notified via the address or phone number provided at the time of entry. If a prize is not claimed within five (5) business days, Sponsors reserve the right in its sole discretion to pick an alternate. No transfer, substitution or cash will be awarded in lieu of stated prizes except in Sponsors’ sole discretion. Sponsors may replace prizes with items of equal or greater value in the event stated prizes are unavailable. Prizes are non-transferable and non-endorsable.

PARTICIPATION:
Participation constitutes entrant's full and unconditional agreement to and acceptance of these Official Rules. All entries become the property of Sponsors and will not be returned. By entering, you agree to these Official Rules and the decision of Sponsors, which are final and binding in all respects.

ADDITIONAL TERMS:
All winners (or their guardians if under 18) must present proper identification. No substitutions will be permitted and prizes may not be exchanged for cash. Any prize not claimed or utilized in accordance with these Official Rules will be forfeited.

MISCELLANEOUS:
The names, logos, and icons identifying the products and services of Sponsors are proprietary marks. The winner acknowledges and agrees that as a condition of winning, if legally permissible, Sponsors have the right to publicize his/her name, likeness, address, and fact that he/she won a prize, and all matters incidental thereto, without further compensation.

All applicable local, state, and federal taxes, if any, and any and all expenses not specified herein, are the responsibility of each winner. All federal, state and local laws apply.

Sponsors and its employees assume no liability or responsibility for the prizes once they have been awarded to the winning entrants. Additionally, they will be held harmless from and against any and all losses, damages, rights, claims and actions of any kind arising in connection with the Award or resulting from acceptance, possession, or use or misuse of any prize awarded in connection with the Award.
